Online Registration Form Each year, the State of Alabama and your county Emergency Management Agency (EMA) identifies people who may need assistance during an emergency such as severe weather, a hazardous material accident or the unlikely event of an accident at Anniston Army Depot. If you or someone in your family qualifies as a person with special needs, your county EMA can provide you with special emergency planning information.

By completing a few simple steps, you can help us identify family members who are unable to evacuate or shelter in place either by themselves or with the assistance of nearby family, friends and neighbors. Please:
  • Read the Self-Registration Form to determine if you should register yourself or a family member.
  • Complete the Self-Registration Form if you or someone in your home:
     
  • Has a physical, mental or medical problem,
  • Needs transportation, or
  • If children are home alone and family, friends or neighbors cannot provide assistance in an emergency.

Do not complete this form if you do not need assistance.
