Online Registration Form
Each year, the State of Alabama and your county Emergency Management Agency (EMA) identifies people who may need assistance in the unlikely event of an accident at the Anniston Army Depot. If you or someone in your family qualifies as a person with special needs, your county EMA can provide you with special emergency planning information and protective equipment.
A key part of personal emergency preparedness, regardless of the type of hazard, is that people understand that they are responsible for establishing their own Help Network of assistance from family, friends or neighbors. In the unlikely event of a chemical weapons agent release, people will have to protect themselves and help each other because county and state emergency services may not be able to respond to the overwhelming number of requests for assistance that would occur. By completing a few simple steps, you can help us identify family members who are unable to evacuate or shelter in place by themselves or with the assistance of nearby family, friends and neighbors. Please: • Review the Self-Registration Form to determine if you should register yourself or a family member. • Print and complete the Self-Registration Form if someone in your home:
• Mail the completed Self-Registration Form to Alabama Emergency Management Agency, PO Box 39, Anniston, AL 36202, or call 256.236.8642 or 800.263.0201 to register by phone. • Do not complete the form if you do not need assistance. This information is being gathered under the authority of the State of Alabama for the purpose of protecting the public. The State will protect your privacy and all confidential information you provide. |
